Request of Direct Payment of TD
1. Termination of the employment relationship during the period of temporary incapacity.

Access the private area of FREMAP Contigo from the WEB or from your mobile/APP to make the direct payment request.
With digital certificate
Log in / register at FREMAP CONTIGO accessing with your electronic certificate.
With user credentials
If you are already a FREMAP user or patient, you can also log in at FREMAP CONTIGO logging in with your username and password. If you are not a user, you will need to create an account first (create your account) HERE).
Press "Application for Direct Payment of TD"
Select the file for which direct payment can be requested and the reason for the direct payment
Add relatives under your responsibility if applicable
Fill in personal income tax details

Attach the documentation required for the application
Fill in the account number where the payments will be made
Review the application and click on “Submit”.
IMPORTANT
Your application will only be considered fully submitted if you have logged in using an electronic certificate; otherwise, you will have to go to any FREMAP office to sign the APPLICATION FORM for it to be valid.
